The Sad Ending is one of the two endings that can be achieved in Airplane 4: One Last Flight, along with the Airplane 4 Good Ending. It occurs if The Players choose to follow Ronald as opposed to General Poncho.
Ronald leads the players to a cave where he says Skeletox hides. The players are attacked by infected, and the players and Ronald flee into the cave to lose their pursuers. Ronald and the players reunite at the end of the cave, in front of an altar of Skeletox, with a statue of Skeletox in front of them. Then the eyes of the statue start glowing red, and the screen fades to white.
The players are then teleported to a flashback of Ronald as a child, set in a black and white hospital. The young Ronald approaches a Doctor, and asks how his parents are. The doctor leads Ronald to an "Emergency Room", where Ronald's mother and father are seen lying on beds. The Doctor says that they will not make it, as the injuries they sustained from the plane crash were too severe. He then leaves to allow Ronald to be alone with them.
A bereaved Ronald is then visited by Skeletox, a hooded skeleton, who tells him he will revive Ronald's parents if he does what he asks. Skeletox then says that a time will come where Ronald must crash a passenger plane, and Skeletox will feast on the souls of the passengers. He also warns Ronald that if he fails his soul will be consumed instead. Ronald agrees to these terms, and would go on to attempt to crash a plane in Airplane 3.
